Home Health Care is an extension of your doctor's care and hospital services into your home. CRMC Home Health Care Services provides trained, compassionate professionals who work with patients and their families to develop and carry out the physician's plan of care. CRMC Home Care Services will stay in close communication with the physician and notify him or her of any changes in the patient's condition or needs. Home Care offers patients the opportunity to recover in the privacy, convenience and familiarity of their own homes. What Services Are Available Under Home Health Care? Chaplain Services CRMC Services was the first home care agency in the state of Alabama to provide chaplain services, and remains the largest faith based agency in the state. Community-Based Care Each Home Care Service is based around a CRMC hospital to ensure community-based care. This allows home care service staff the ability to personally know the patient's doctors and nurses in order to provide the most accurate care available. Who Qualifies For Home Care Under Medicare? To qualify for home health under Medicare a person must be homebound, under the care of a physician, and in need of skilled services on an intermittent basis. Intermittent home care services are those that are only needed a few hours a day or week. A patient does not have to be bedridden or totally confined to home to qualify for Medicare reimbursement. The patient is considered "homebound" if he or she is unable to leave home without considerable and taxing effort. A physician must establish and periodically review the patient's plan of care. Who Pays For Home Care Services? Many insurance plans cover home care services, including Medicare. CRMC Home Care Services personnel will file all insurance claims. Medicare covers most of the Home Care Services cost. Most major medical insurance plans reimburse up to 80% for home care. Some patients may also have coverage through a HMO or preferred provider organization (PPO). CRMC Home Care Services will conduct an assessment to determine what services are covered by your particular plan and that plan's eligibility requirements. Patients who may benefit form Home Care Services are: Patients recovering from surgery or illness Patients with cancer or long-term disease Homebound individuals requiring skilled intermittent services Patients requiring high-tech procedures such as IV therapy or ventilator care Anyone with a chronic condition which requires frequent monitoring Patients requiring therapy to overcome injuries or an illness The terminally ill in need of medical care and emotional supportHow Do You Secure Home Care Services?
