Cullman Regional Medical Center Case Management & Discharge Planning Services are here to help you and your family make arrangements for care following your hospitalization. All of our Case Managers are Registered Nurses with experience in nursing and discharge planning.
Location
Our office is located on the second floor of Professional Office Building 2; however, our Case Managers come and meet with patients beside to help make plans for care after their hospital stay.
Hours of Operation
Discharge Planning services are available seven days a week as needed by patients.
