Diabetes Care Center Staff & Support


The CRMC Diabetes Care Center has been assisting diabetic patients since 2001. The Center was recently awarded Education Recognition by the American Diabetes Association for the third time since its opening. The American Diabetes Association Education Recognition program has grown to become the leading quality assurance mechanism for diabetes self-management education programs across the country. Programs that achieve this distinction 

The staff consists of one  Registered Dietitian with more than 20 years experience and one certified nurse practitioner with 23 years of nursing experience. All staff members are certified diabetes educators. Certified Diabetes Educators are required to have a minimum of 1,000 hours in diabetes self-management experience to apply for certification and are required to re-certify every five years. 

Our staff can assist you with meal planning, self glucose monitoring, medication tips, etc. Physician referrals are required to enroll in the education program. Call the CRMC Diabetes Care Center office for more information at (256) 737-2025. 

Meet the CRMC Diabetes Educators

Kim Addison, RD, CDE
  Kim Addison, RD, CDE, Program Coordinator
Medical Nutrition Therapist Kim Addison has been assisting diabetic patients through diabetes education for more than 25 years in both inpatient and outpatient settings. She has been a part of the CRMC Diabetes Care Center Team for the past eight years. Addison received her Bachelor of English degree from the University of South Alabama, her Bachelor of Nutrition degree from the University of Alabama and her Masters in Education from the University of South Alabama. Addison is a Certified Insulin Pump Trainer, a Certified Diabetes Educator and is certified in Childhood and Adolescent Weight Management.
  
     

  Lisa Ellard, CRNP, CDE
Lisa Ellard has been caring for patients as a nurse for more than 23 years and assisting diabetic patients for more than 15 years. Ellard received her Bachelor of Science in Nursing degree from Auburn University where she served as Vice President of the Auburn University School of Nursing Program. Lisa began her career as a nurse in Birmingham but moved to Cullman Regional’s Maternity Center shortly after to be a Labor, Delivery, Postpartum and Gynecology Registered Nurse. While working as a nurse, Ellard received her Master of Science in Nursing degree from the University of Alabama in Huntsville. Shortly after receiving her masters and then certification, Ellard began seeing patients as a Certified Registered Nurse Practitioner (CRNP) with Cullman Internal Medicine. It was during this time that Ellard became interested in diabetes education and became a Certified Diabetes Educator. Ellard has served as a clinical nursing instructor for Wallace State Community College in Hanceville and has served and continues to serve as a Diabetes Educational Speaker. She recently joined the clinical team of Hospice of Cullman County and includes Michael Hall, MD and William Peinhardt, MD. This clinical team visits hospice patients in their homes while under the care of Hospice of Cullman County. Ellard has been a part of the Diabetes Care Center Team since 2001 and has been serving as the Diabetes Care Center Coordinator since 2001. Lisa assists patients one-on-one with their diabetes by appointment.
     


Adult Diabetes Support Group


Our staff coordinates a monthly Adult Diabetes Support Group that is open to anyone interested in learning more about Adult Diabetes. The group meets the fourth Thursday of each month at 6 pm in the in the CRMC Community Education Center in Professional Office Building 2.
