Nursing Care by Registered Nurses
  • Manages patient’s medical problems
  • Educates patients and their family members on the primary physician’s care plan
  • Communicates with primary physician on patients status
  • On-call for medical emergencies 24-hours a day, 7 days a week
Home Health Aide Service
Personal care assistance for the patient in the home which could include
  • Provides skin, foot, mouth and nail care
  • Bathes the patient
  • Assists with dressing activities
  • Changes bed linens
Physical Therapy
Therapeutic services to assist patients with weakness or functional decline through therapeutic exercise to improve strength. The therapist will evaluate the patient for specific needs, evaluate the home for safety and mobility and educate the patient in the proper use of equipment. These exercises are designed to restore the patient to an appropriate level of functional independence.

Nutritional Support


Chaplain Services
  • Provides spiritual and emotional support to patient and family
  • Communicates with family minister
  • Assists with funeral arrangements, as needed
Social Services
  • Refers patients and family members to community service organizations that can provide assistance for various needs
  • Assists with financial and legal matters
Volunteers
  • Relieves caregivers
  • Provides transportation
  • Shops/runs errands as needed
  • Provides a listening ear, a shoulder to cry on, meals, etc.
